Serenity Hospice is seeking a compassionate and dedicated Registered Nurse (RN) case manager to join our growing team serving the Hazleton/Weatherly, Pennsylvania area. Responsibilities:
Provide skilled nursing care to hospice patients in their homes or care facilities
Perform patient assessments and develop individualized care plans
Collaborate with the interdisciplinary hospice team
Educate patients and families on disease progression and comfort care
Maintain accurate and timely clinical documentation
Qualifications:
Active Pennsylvania RN license
Hospice or home health experience preferred, but not required
Strong assessment and communication skills
Compassionate approach to patient and family care
Valid driver’s license and reliable transportation
