Certified Registered Nurse Practitioner (CRNP) – Hospice
Primarily InOffice | Regional Team | No Nights or Weekends
Serenity Hospice is seeking a compassionate and experienced Certified Registered Nurse Practitioner (CRNP) to join our dedicated regional hospice team. This is a primarily inoffice position focused on meaningful, virtual and actual facetoface clinical visits with hospice patients while collaborating closely with physicians and interdisciplinary care teams.
In this role, you will play a vital part in ensuring exceptional endoflife care by conducting required hospice facetoface evaluations and supporting highquality, compliant, and compassionate patient care across our service region.
Position Highlights

Key Responsibilities
Conduct facetoface hospice patient visits to evaluate continued eligibility in accordance with CMS Hospice Eligibility Guidelines
Collaborate with the Hospice Medical Director and attending physicians to review patient status and adjust care plans as appropriate
Provide clinical leadership and guidance to the interdisciplinary hospice team
Support symptom management strategies and clinical decisionmaking
Authorize medications and treatments in accordance with state regulations and patient care needs
Ensure accurate, timely, and compliant clinical documentation
Participate in Interdisciplinary Group (IDG) meetings and care planning discussions
Work primarily from an assigned Serenity Hospice office, with occasional local travel to patient homes or care facilities
Qualifications
Current and active CRNP license in Pennsylvania
Hospice or palliative care experience required
Thorough understanding of CMS Hospice Eligibility Guidelines
Strong interpersonal, organizational, and documentation skills
Ability to work collaboratively within an interdisciplinary team
Willingness to travel locally within the region, as needed
