Responsibilities
Clinical Leadership
Provide day-to-day leadership and support to hospice RNs, LPNs, and hospice aides
Guide care planning and clinical decision-making
Serve as a clinical resource for symptom management, pain control, and end-of-life care
Review clinical documentation for accuracy and regulatory compliance
Patient Care Oversight
Participate in admissions, recertifications, and complex patient visits as needed
Support staff with challenging cases, family communication, and symptom escalation
Coordinate care with the interdisciplinary team, including physicians, social workers, and chaplains
Team Management
Coach, mentor, and provide feedback to nursing staff
Assist with scheduling, staffing coverage, and workload distribution
Facilitate team meetings, case conferences, and clinical updates
Quality and Compliance
Ensure adherence to Medicare Conditions of Participation and state regulations
Support survey readiness, audits, and quality improvement initiatives
Monitor quality indicators, including hospitalizations, symptom control, and visit utilization
Communication
Serve as liaison between field staff, leadership, and community facilities
Communicate changes in patient condition and plan of care
Support patient and family education and conflict resolution

Qualifications:
- Graduate from an accredited School of Nursing.
- Current licensure as a Registered Nurse (RN) in the state.
- Strong assessment and problem-solving skills.
- Proficient in organization, time management, and understanding case management processes.
- Valid driver's license with access to a reliable, insured vehicle in good working condition.
- Excellent verbal and written communication skills.
- Clear RN license with no restrictions; must be able to administer medications, perform medication reconciliations, and provide nursing procedures or treatments within the scope of state nursing practice.
- Completion of competency assessments prior to patient care and as needed for less common procedures or treatments.
- Hospice and palliative care experience preferred (2–5 years)
- Leadership or supervisory experience preferred
- Strong knowledge of symptom management and end-of-life care
