Hospice Admissions RN

Serenity Hospice and Home is seeking a compassionate and dedicated Hospice Admissions Registered Nurse (RN) to join our team. If you are passionate about providing exceptional patient-centered care and helping patients and families navigate one of life’s most important transitions, we would love to hear from you.

As a Hospice Admissions RN, you will play a vital role in ensuring patients and families receive a seamless, supportive, and compassionate transition to hospice care. Admissions, family meetings, and patient care may take place in our eight-bed inpatient unit or in patients’ homes.

Schedule

  • Monday–Friday, 9:00 a.m.–5:30 p.m.
  • Participate in the on-call team one Saturday and one Sunday every four weeks
  • A scheduled day off before and after the on-call weekend
  • Mileage reimbursement provided for authorized business travel

Key Responsibilities

  • Conduct comprehensive nursing assessments of patients referred for hospice services and determine their clinical needs for hospice admission.
  • Coordinate and facilitate timely, compassionate hospice admissions in collaboration with physicians, patients, families, and the interdisciplinary team.
  • Participate in family meetings and provide clear, compassionate education regarding hospice services, the plan of care, medications, symptom management, and what families can expect.
  • Develop individualized plans of care that reflect each patient’s goals, preferences, values, and clinical needs.
  • Provide skilled nursing care and support to patients in their homes and, when needed, in our inpatient unit.
  • Collaborate closely with physicians, social workers, chaplains, hospice aides, bereavement staff, and other members of the interdisciplinary team to ensure continuity of care.
  • Respond to patient and family needs with compassion, professionalism, and clinical expertise.
  • Maintain accurate, timely, and complete clinical documentation in accordance with organizational policies, regulatory requirements, and professional standards.
  • Promote a positive hospice experience by advocating for patients and families throughout the continuum of care.

Qualifications

  • Current Registered Nurse (RN) license in good standing in Illinois.
  • Hospice or palliative care experience preferred, but not required.
  • Strong clinical assessment, critical-thinking, and decision-making skills.
  • Excellent communication and interpersonal skills.
  • Ability to provide compassionate, respectful, and patient-centered care.
  • Strong organizational and time-management skills with attention to detail.
  • Ability to work independently in patients’ homes while also collaborating effectively with the interdisciplinary team.
  • Valid driver’s license, reliable transportation, and the ability to travel within the service area.
