What are the responsibilities and job description for the Hospice Admissions RN position at Serenity Hospice and Home?
Serenity Hospice and Home is looking for compassionate and dedicated Registered Nurses with a passion for providing exceptional care! We are seeking to hire two Hospice Admission RNs to join our caring team and make a difference in the lives of patients and their families. One of the positions would work from 8 am to 5 pm, and the other would work from 10 am to 7 pm, Monday through Friday, while assisting the on-call team on one Saturday and one Sunday every 5 weeks, taking a day off before and after those weekends. Admissions, family meetings, and patient care may take place in our eight-bed inpatient unit or in a patient's home. Mileage is reimbursed.
Key Responsibilities:
- Conduct comprehensive assessments of patients referred to hospice care, ensuring a seamless and supportive admission process.
- Collaborate with interdisciplinary teams to develop personalized care plans that honor the unique needs and wishes of each patient.
- Provide education and emotional support to patients and their families, helping them navigate the transition to hospice care with dignity and understanding.
- Maintain accurate and timely documentation in accordance with regulatory and organizational standards.
Qualifications:
- Current RN license in good standing.
- Experience in hospice or palliative care preferred, but not required.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- A compassionate and empathetic approach to patient care.
- Detail-oriented individual who is highly organized.
