Duties
- Assist clients with daily activities, including personal hygiene, meal preparation, and mobility support
- Provide emotional support and encouragement to clients.
- Monitor vital signs, administer medications as directed, and document medical records accurately in electronic health records (EHR) systems while maintaining patient confidentiality in accordance with HIPAA regulations
- Provide crisis management and de-escalation techniques during behavioral or emotional episodes to ensure safety for clients and staff
- Support client individual plan of care through ongoing data collection, and progress documentation
- Collaborate with family, social workers, healthcare providers, and families to coordinate comprehensive client care plans
- Conduct first aid and CPR when necessary, ensuring immediate response to emergencies
Experience
- Prior experience working elderly and disabled population in home settings is highly desirable
- Knowledge of knowing when to report changes in status
- Familiarity with medical terminology, medical documentation practices, and electronic health records (EHR) management is required
- Private sitting or nursing home experience are a plus
- Strong communication skills for client interaction, team collaboration, and family engagement are vital
- Certifications such as first aid/CPR are a plus
