Job Overview
We are seeking a dedicated and experienced Registered Nurse Case Manager to join our hospice team. The ideal candidate will possess a strong clinical background and be skilled in managing patient care across various settings. This role involves coordinating care for patients, ensuring they receive appropriate services, and facilitating their transition through the healthcare system. The Registered Nurse Case Manager will work closely with patients, families, and healthcare providers to optimize health outcomes.
Duties
- Conduct comprehensive patient assessments to determine and develop individualized care plans.
- Coordinate and manage patient care of hospice patients.
- Collaborate with interdisciplinary teams (IDG) to ensure continuity of care and effective discharge planning.
- Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
- Educate patients and families about their conditions, treatment options, and self-management strategies.
- Maintain accurate medical records in compliance with HIPAA regulations and ensure all documentation is up-to-date.
- Utilize critical thinking skills to evaluate diagnostic information and make informed decisions regarding patient care.
- Engage in utilization management to ensure appropriate resource allocation while maintaining high-quality patient service.
- Provide support for patients with complex needs including those requiring infectious disease care, geriatrics, behavioral health, and pediatrics.
Requirements
- Current RN license in good standing.
- Hospice experience preferred.
- Proven experience in critical care, ICU, emergency medicine, or hospital medicine is preferred.
- Familiarity with medical terminology, discharge planning, case management principles, and utilization management practices.
- Knowledge of hospice physiology and ability to perform patient assessments including vital signs monitoring.
- Experience in managing diverse patient populations including adults.
- Strong communication skills for effective collaboration with healthcare teams and patient education.
- Ability to navigate electronic medical records systems efficiently.(HCHB)
