Job Overview
We are seeking a dynamic and compassionate Registered Nurse Case Manager to join our dedicated hospice team for our King George office. In this vital role, you will coordinate comprehensive patient care across various settings, ensuring seamless transitions from inpatient to outpatient and post-acute care. Responsibilities
- Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to inform individualized care plans.
- Manage inpatient and outpatient caseloads across diverse clinical environments such as hospital medicine, emergency rooms, PICU (Pediatric Intensive Care Unit), Geriatrics, behavioral health, and surgical units.
- Coordinate discharge planning by collaborating with multidisciplinary teams to ensure timely and appropriate post-acute care arrangements.
- Facilitate diagnostic evaluations and utilization management processes to optimize patient care pathways while adhering to HIPAA regulations and medical records protocols.
- Perform patient intake procedures, including medical terminology documentation and comprehensive patient service assessments.
- Engage in case management activities focused on hospital experience, managed care, outpatient clinics, and post-acute settings to improve patient outcomes.
- Utilize strong physiology knowledge and medical-surgical expertise to support complex cases involving infectious disease care, trauma centers (Level I & II), emergency medicine, and other specialized fields.
- Maintain accurate documentation of patient records, medical histories, and discharge summaries while ensuring compliance with all regulatory standards.
Qualifications
- Valid RN license with current registration in the applicable state or jurisdiction.
- Hospice Experience preferred.
- Strong understanding of medical terminology, physiology principles, diagnostic evaluation techniques, and discharge planning processes.
- Experience with utilization management programs and managed care systems to coordinate effective patient services. (HCHB)
- Excellent skills in patient assessment, intake procedures, vital signs monitoring, and medical records documentation.
- Knowledge of HIPAA compliance standards and best practices for safeguarding patient information.
