Job Overview
We are seeking a compassionate and dedicated On-call RN Hospice Case Manager to join Serenity First Hospice in King George area. Duties
- This position will be On-Call for our Hospice company, King George OFfice. Will take after hour calls and will triage or if necessary conduct a visit with the Patient.
- Conduct thorough patient assessments, including vital signs, and medical history reviews to determine care needs
- Develop and implement individualized hospice care plans in collaboration with interdisciplinary teams, including physicians, nurses, social workers, and therapists
- Coordinate discharge planning and post-acute care transitions to ensure seamless service delivery across inpatient, outpatient, and clinic settings
- Manage intake processes for new patients, ensuring compliance with HIPAA regulations and medical records documentation standards
- Oversee utilization management to optimize resource use while maintaining high-quality patient care standards
- Conduct patient education on physiology, medication management, symptom control, and advance directives to empower patients and families
- Assist with outpatient clinic visits and outpatient post-acute services to ensure continuity of care after hospitalization or emergency room visits
- Maintain accurate medical records and documentation of patient interactions in compliance with healthcare regulations
Experience
- Proven experience in hospice preferred
- Critical care experience with familiarity in managing hospice patients
- Experience working within managed care organizations or utilization management programs is highly desirable
- Strong background in patient assessment, discharge planning, diagnostic evaluation, and case management across inpatient and outpatient settings
- Knowledge of medical terminology, physiology principles, HIPAA compliance standards, and medical records documentation practices
- Prior experience in geriatrics/hospice settings is advantageous for supporting diverse patient populations
- Demonstrated ability to coordinate multidisciplinary teams across hospital experience including infectious disease care and OR experience where applicable
- Excellent communication skills for engaging with patients of all ages—including pediatrics—and their families during emotionally sensitive times
