We are seeking a dedicated and compassionate Hospice Registered Nurse Case Manager to provide comprehensive patient care within a hospice setting. The ideal candidate will possess a strong background in nursing, with expertise in managing complex health conditions, coordinating multidisciplinary care, and ensuring comfort and dignity for patients facing end-of-life stages. As a key member of our healthcare team, the Hospice RN Case Manager will deliver personalized care plans, support families, and uphold the highest standards of clinical excellence and compliance with health regulations.
Duties
- Assess patients’ health status through detailed evaluations, including physiology and anatomy knowledge, to develop individualized care plans.
- Coordinate interdisciplinary team efforts to deliver holistic hospice care tailored to each patient’s needs.
- Monitor and document patient progress using electronic health records (EHR) and medical documentation standards, ensuring accuracy and confidentiality in compliance with HIPAA regulations.
- Administer medications, manage symptom control, and provide emergency care when necessary, including first aid and CPR procedures.
- Educate patients and families on disease management, pain control, and disease prevention strategies to enhance quality of life.
- Conduct triage assessments to prioritize urgent needs and facilitate timely interventions in acute or chronic care situations.
- Ensure adherence to health regulation policies while maintaining compliance with medical records management and confidentiality protocols.
- Provide nursing leadership by mentoring staff, overseeing medical documentation processes, and promoting best practices in medical terminology application.
Requirements
- Valid nursing license with current registration in the applicable state or region.
- Proven experience in hospice or palliative care nursing, with a solid understanding of chronic disease management and acute care procedures.
- Proficiency in medical software systems, EMR/EHR management, and electronic health records documentation.
- Strong knowledge of health regulation policies such as HIPAA and safety standards related to patient privacy and confidentiality.
- Certification in Basic Life Support (BLS), CPR, and First Aid is required; additional certifications in emergency care are preferred.
- Familiarity with medical terminology, physiology, anatomy, and disease processes relevant to end-of-life care.
- Demonstrated ability to handle confidential information professionally while providing compassionate patient-centered care.
- Excellent communication skills with the ability to educate patients’ families effectively while coordinating multidisciplinary teams.
- Leadership experience within nursing teams is advantageous for fostering a collaborative work environment.
