Job Summary
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including hospitals, nursing homes, and assisted living facilities. The LVN will work under the supervision of registered nurses and physicians, assisting with patient assessments, administering medications, and ensuring the comfort and well-being of patients.
Duties
- Conduct patient assessments and document vital signs accurately.
- Administer medications and treatments as prescribed by healthcare providers.
- Assist in the development and implementation of individualized care plans.
- Provide wound care, IV infusion, catheter care, and other nursing procedures.
- Monitor patients' conditions and report any changes to the supervising nurse or physician.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Educate patients and their families on health management and disease prevention.
- Maintain accurate medical records in compliance with HIPAA regulations.
- Participate in discharge planning and case management for patients transitioning to home or other care settings.
Skills
Candidates should possess a diverse range of skills to excel in this role, including but not limited to:
- Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
- Experience in critical care, acute care, long-term care, assisted living, or hospice environments.
- Knowledge of medical terminology, anatomy, physiology, ICD-9/ICD-10 coding, and CPT coding.
- Strong patient assessment skills with experience in triage and emergency medicine.
- Ability to provide specialized care for individuals with developmental disabilities, dementia, Alzheimer's disease, or other memory-related conditions.
- Competence in performing procedures such as venipuncture, phlebotomy, tube feeding, suturing, and airway management.
- Familiarity with infection control practices and aseptic techniques.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
