Position Summary
Serenity Congregate Care is seeking a PRN Contract Social Worker to provide intermittent social services support for our Congregate Living Health Facility (CLHF). This position is ideal for a social worker looking for flexible supplemental work and will be utilized on an as-needed basis to assist with resident assessments, discharge planning, family support, and community resource coordination.
Responsibilities
- Conduct psychosocial assessments as requested.
- Assist with discharge planning and community resource referrals.
- Provide support and counseling to residents and families.
- Participate in care planning meetings as needed.
- Assist with resident advocacy, grievance resolution, and family communication.
- Coordinate with nursing staff, physicians, case managers, and outside agencies.
- Complete required documentation in accordance with facility policies and regulations.
Qualifications
- Bachelor's Degree in Social Work (BSW) preferred.
- Previous experience in healthcare, skilled nursing, rehabilitation, hospital, or long-term care preferred.
- Knowledge of discharge planning and community resources.
- Strong communication and documentation skills.
- Ability to work independently with minimal supervision.
