Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts or other assistive devices.
- Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates to track health status accurately.
- Administer medications following medical documentation protocols and ensure adherence to HIPAA regulations for patient privacy and confidentiality.
- Support patients with specialized care needs such as Alzheimer's disease, dementia, hospice care, or memory care by providing compassionate companionship and supervision.
- Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient interactions, medical records, and care plans precisely.
- Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment conducive to recovery or ongoing care.
- Work collaboratively with healthcare professionals to deliver comprehensive home & community care tailored to each individual’s needs.
Experience
- Prior experience in home health or home care settings is highly preferred, especially in assisted living facilities or nursing homes.
- Demonstrated ability to work with individuals with developmental disabilities or disabilities requiring specialized support.
- Knowledge of medical terminology and procedures related to long-term care, hospice, dementia, and Alzheimer's care.
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as catheter care or medication administration are advantageous.
- Experience operating medical equipment like Hoyer lifts and taking vital signs accurately is essential.
- Familiarity with HIPAA compliance standards and medical documentation practices ensures high-quality patient care while maintaining confidentiality.
