Job Summary
The Street Outreach Supervisor is responsible for developing and implementing strategic community outreach initiatives to identify, engage, and enroll eligible members in the Enhanced Care Management (ECM) and Community Supports (CS) programs in their respective area. This role requires strong leadership skills, community relationship building, and the ability to work independently while meeting measurable performance targets.

Job Summary
The Street Outreach Supervisor is responsible for developing and implementing strategic community outreach initiatives to identify, engage, and enroll eligible members in the Enhanced Care Management (ECM) and Community Supports (CS) programs in their respective area. This role requires strong leadership skills, community relationship building, and the ability to work independently while meeting measurable performance targets.
Supervisory Responsibilities
- Effectively supervise a team of community outreach representatives, ensuring productivity goals are met.
- Assist with progressive discipline documentation and meetings as needed.
- Supervise timecards for staff as needed.
- Identify and establish relationships with key community locations, including but not limited to homeless shelters, community health centers, social service agencies, libraries, food banks, and other high-need areas.
- Identify areas with high concentrations of unhoused populations who would benefit from the ECM and CS programs, including encampments, transit hubs, service corridors, and other gathering locations.
- Develop and implement regular visiting schedules for identified locations with potential ECM and CS program participants.
- Develop and maintain a comprehensive mapping system of outreach locations and community resources.
- Build partnerships with community-based organizations, healthcare providers, and social service agencies.
- Facilitate enrollment processes and ensure proper documentation and consent procedures.
- Collaborate closely with direct manager to align outreach strategies with organizational goals.
- Meet established daily, weekly and monthly enrollment targets and performance metrics.
- Track and analyze outreach data to identify trends and opportunities for improvement.
- Prepare daily, weekly and monthly reports on outreach activities, enrollment numbers, and community engagement outcomes.
- Develop strategies to address performance gaps and optimize outreach effectiveness.
- Perform other duties as assigned.
- High school diploma or GED required. Bachelor's degree in Social Work, Public Health, Healthcare Administration, or related field preferred.
- Minimum 2 years of experience in community outreach, case management, or healthcare program coordination.
- Previous supervisory or leadership experience required.
- Experience working with vulnerable populations, including individuals experiencing homelessness, mental health challenges, or substance use disorders.
- Strong understanding of social determinants of health.
- Experience with ECM / CS and CalAIM preferred.
- Strong interpersonal and communication skills with ability to engage diverse populations.
- Excellent organizational and time management abilities.
- Proficiency in data collection, analysis, and reporting.
- Knowledge of community resources and social services.
- Cultural competency and sensitivity when working with diverse communities.
- Ability to work independently and manage multiple priorities.
- Valid California driver's license and reliable transportation.
- Bilingual capabilities (English/Spanish) strongly preferred.
- Knowledge of healthcare regulations including HIPAA compliance requirements.
- Prolonged periods sitting at a desk and working on a computer.
- Must be able to lift up to 15 pounds at times.
- May require occasional travel between facilities.
- Ability to work in various community settings, including outdoor environments.
- Walking and standing for extended periods during outreach activities.
- Ability to travel throughout the general areas around the community.
