Job Summary:
Therapy professionals at Seraphic Springs Health Care Agency, Inc. play a vital role in delivering compassionate, rehabilitative care to patients in the comfort of their homes. Under the supervision of a Clinical Supervisor or Therapist, Physical Therapists (PT), Occupational Therapists (OT), Physical Therapist Assistants (PTA), and Certified Occupational Therapy Assistants (COTA) work to improve each patient’s mobility, independence, and quality of life through personalized treatment plans.
We are looking for dedicated and licensed therapy professionals who are passionate about serving in the home health setting and who uphold our mission of providing Quality, Affordable, Home Health Care.
Qualifications:
Must have a current and valid license to practice in the state of Massachusetts
Graduate of an accredited therapy program (PT, OT, PTA, or COTA)
CPR certification (current and active)
Minimum of one (1) year of experience in a clinical or home health care setting is preferred
Strong assessment, documentation, and communication skills
Ability to work independently and collaboratively within an interdisciplinary team
Reliable transportation and willingness to travel for home visits
Must pass background checks and provide professional references
EMR experience preferred (but not required)
Primary Duties/Responsibilities:
Conduct thorough evaluations of patients and develop individualized treatment plans (PT/OT)
Provide therapy services according to the care plan, focusing on functional outcomes and patient safety
Assist in restoring function, improving mobility, relieving pain, and preventing physical disabilities (PT)
Promote and facilitate independence in activities of daily living (OT)
Implement and document progress of therapy interventions and adjustments (PTA/COTA under supervision)
Educate patients and caregivers on safe home exercises and assistive device usage
Collaborate with the care team, including RNs and caregivers, to ensure coordinated care
Monitor patient response to treatment and report changes to the supervising therapist or physician
Maintain accurate and timely documentation in compliance with agency and state standards
Participate in team meetings, case conferences, and ongoing training as needed
