Overview:

The Registered Nurse will work with a multidisciplinary team to provide individualized person-centered services to adults with a severe mental illness. This is a community-based program with flexibility to go into the community or client’s home to provide care. The work includes individual case management, group counseling, medication and medical procedure skills training, and coordination with medical providers to assist the client in achieving identified goals, improving health and wellness and reducing hospital and jail stays.

Essential Job Functions:

  • Participate in implementing and maintaining nursing standards
  • Participate in determining resources and policies essential to the delivery of nursing care services
  • Assess medical care needs and other services to support wellness
  • Recommend health objectives for recipients’ service plans
  • Facilitate consumer education on prevention, health, wellness, diet, HIV/AIDS education, smoking cessation and recovery
  • Provide education to program staff, including residential staff, regarding psychiatric medications, physical health issues and medical care protocols
  • Provide consultation, medication training and support, as well as crisis intervention services as needed.
  • Act as advocate and liaison for recipients in securing medical care, and other community supports that promote integrated physical and mental health wellness
  • Review and audit medication management procedures and teach program staff, including residential staff, how to follow and adhere to the procedures
  • Review MAR, orders, and other pertinent information at Residential Facilities quarterly and as needed
  • Assist LMP with care coordination between mental and physical health.
  • Assist with VS check as directed by LMP
  • Administer IM medications and provides and reads PPD tests.
  • Coordinates with pharmacies to ensure that recipients medications are filled and available in reasonable time frame.
  • Other duties as assigned by supervisor

Essential Job Requirements:

  • Registered Nurse (RN) with current Oregon State license
  • At least three years of experience working with adults with serious mental illness and co-occurring substance use disorders
  • Experience understanding various models and theories pertaining to treatment of serious mental illness and risk management
  • Extensive knowledge of healthcare, community treatment, support services and resources
  • Ability to work effectively with diverse team and remain highly organized and self-motivated
  • Proficient in computers and fluency in Spanish preferred

Physical requirements:

  • Ability to meet the following physical requirements with or without reasonable accommodation:
  • Move or operate equipment and objects.
  • Navigate and position oneself within the workspace.
  • Ability to lift 20 lb.;
  • Remain in a stationary position (standing or sitting) for extended period
  • Communicate effectively with clients, both verbally and in writing;
  • Review, interpret, and produce written documents
