Family Nurse Practitioner (FNP)
Primary Care & Geriatrics | Onalaska, La Crosse & La Crescent Area
Sequoia Integrative Medical Services LLC is seeking a compassionate, motivated, and patient-focused Family Nurse Practitioner to join our growing healthcare team serving the Onalaska, La Crosse, and La Crescent communities.

What You'll Do
- Conduct comprehensive patient assessments, physical examinations, and routine follow-up visits.
- Diagnose, treat, and manage acute and chronic medical conditions, including diabetes, hypertension, COPD, asthma, and other common illnesses.
- Complete admission history and physical examinations.
- Perform medication reconciliation during admissions, consultations, and discharges.
- Develop individualized treatment plans focused on prevention, wellness, and chronic disease management.
- Educate patients, families, and caregivers on treatment plans and healthy lifestyle practices.
- Collaborate with physicians, nursing staff, therapists, and interdisciplinary care teams to ensure coordinated, high-quality care.
- Support patients' behavioral health needs through compassionate, whole-person care.
- Maintain accurate and timely clinical documentation in accordance with regulatory and organizational standards.
Qualifications
- Master's or Doctorate degree in Nursing from an accredited program.
- Current Family Nurse Practitioner certification.
- Active Wisconsin Nurse Practitioner license (or ability to obtain prior to start date).
- DEA registration (or ability to obtain prior to start date).
- Excellent clinical judgment, communication, and interpersonal skills.
- Experience in primary care, geriatrics, skilled nursing, or long-term care is preferred but not required.
- New graduates with strong clinical training and a passion for geriatric care are encouraged to apply.
