Schedule: On Call – Nights (5:00pm – 8:00am) or Weekends (8:00am – 8:00pm)

We are currently seeking experienced and empathetic On-Call Admissions Registered Nurses (RNs) to join our team. This vital role supports patients and families during evenings and weekends, initiating hospice services with professionalism and compassion.
Role Summary
The On-Call Admissions RN is responsible for completing hospice admissions during night and weekend hours. This includes assessing eligibility, educating families, and initiating care plans to ensure a smooth transition into hospice services.
Key Responsibilities
- Conduct timely hospice admission assessments in homes, facilities, or hospitals.
- Determine eligibility based on Medicare guidelines and agency protocols.
- Educate patients and families on hospice philosophy, services, and expectations.
- Complete all required admission documentation, including informed consent.
- Initiate individualized care plans and communicate with the interdisciplinary team.
- Collaborate with physicians, case managers, and referral sources.
- Provide emotional support and reassurance during the admission process.
- Maintain accurate and timely documentation in the EMR system.
- Respond to urgent admissions and assist with triage as needed during on-call hours.
Qualifications
- Active California RN license.
- Minimum 2 years of nursing experience (hospice, palliative care, or home health preferred).
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently during nights and weekends.
- Valid driver’s license, reliable transportation, and auto insurance.
- Proficiency with EMR systems and documentation standards.
