Position Summary
As a Certified Hospice Aide (HHA) , you will be a vital member of our interdisciplinary care team, working under the supervision of a registered nurse to provide personal care and support to patients in their homes. You’ll help ensure comfort, dignity, and safety for patients during their end-of-life journey.
Key Responsibilities
- Provide personal care including bathing, grooming, dressing, toileting, and skin care.
- Assist with ambulation, exercise, and range-of-motion activities as directed.
- Plan and prepare nutritious meals; assist with feeding as needed.
- Take and record vital signs (temperature, pulse, respiration, blood pressure) when ordered.
- Support patients with self-administration of medications.
- Maintain a clean and safe living environment for patients.
- Perform light housekeeping and laundry duties as appropriate.
- Observe and report changes in patient condition to the supervising nurse.
- Attend interdisciplinary case conferences and contribute to care planning.
- Maintain accurate documentation and adhere to agency standards.
Qualifications
- Active California Home Health Aide Certification.
- Current CPR certification.
- At least 18 years of age.
- Ability to work independently with minimal supervision.
- Empathy and compassion for patients and families.
- Reliable transportation, valid driver’s license, and auto insurance.
- Physical ability to lift, transfer, and assist patients; prolonged standing and walking required.
