What are the responsibilities and job description for the Transplant Coordinator, RN - Renal Transplant position at Sentara Health?
Sentara Healthcare is seeking a Transplant Coordinator for our Kidney/Pancreas Transplant team at Sentara Norfolk General Hospital.
Shift: Days M-F
Outpatient clinic is open Monday – Friday between 8am and 4:30pm. Transplant Coordinator will have some scheduled call and have weekend call about every 11th week.

What you will do:
Coordinate care with post-transplant patients.
Facilitates clinic flow, transplant evaluation, and patient care management.
Provide weekly education sessions to patients and support member. Arranges appointments with surgeons, social worker, financial coordinator, pharmacy and dietary.
Transplant Coordinator admits patient, provides support to patient/caregiver during the waiting period, and discharges transplant patient with instructions for going home.
Transplant Coordinator will manage a case load of patients, remain patient’s point of contact through the transplant process, recovery and at home.
Requirments:
- BSN required
- should have 2+ years of acute care experience, good medical - renal knowledge, and understands chronic disease.
- BLS required.
- Certified Clinical Transplant Coord (CCTC) required within three (3) years of hire.
