Advanced Practice Provider (APP) - NP or PA - Hospital Medicine & Discharge Transition Clinic (Hybrid Role)
Position Summary
The APP supports patients across the continuum of care through a hybrid role that includes both inpatient hospital medicine practice and ambulatory post-discharge transitional care.
This position is designed to strengthen continuity between hospitalization and recovery by providing short-term follow-up care in the Discharge Transition Clinic during the critical weeks after hospital discharge, while also maintaining clinical practice within the hospital medicine service.
The APP will help prevent avoidable readmissions, reinforce discharge plans, close care gaps, and ensure safe handoffs back to primary care providers. The role requires strong inpatient clinical experience, excellent communication skills, and comfort practicing across care settings.

- Part-time practice in the Discharge Transition Clinic (Sentara Family Medicine - Hampton Family Practice)
- Part-time inpatient rounding responsibilities with Sentara Hospital Medicine (SCH)
- Collaboration with hospitalists, PCPs, specialists, case management, health plan, and ambulatory partners
- Participation in transition-of-care quality improvement efforts

Salary : $5,250 - $10,000
