Education
Master’s Degree – Social Work (Required)
Certification/Licensure
- Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI within 90 days of hire (Required)
- Certified Case Manager (CCM) - Certification - Commission For Case Manager Certification (CCMC) within 1 year of hire (Required)
- 1 year of case management experience (Required)
- Experience in hospital setting (Preferred)
- Experience with discharge planning (Preferred)
- Collaborates with patients, families, healthcare team members, and community organizations, and using strong advocacy skills identifies needs and resources in the hospital and community.
- Develop and evaluates effective transition plans taking into consideration the client diagnosis, prognosis course of treatment, past and present services, short-term and long-term goals, provider options, and/or available health care benefits as appropriate to the continuum of care.
