Position Status: Full Time - Day Shift
Position Location: Remote with occasional onsite meetings and trainings in Chesapeake, VA.
Hourly Range: $17.31 - $28.80

Overview
Evaluating and processing referrals by receiving and responding to incoming calls from referral sources or potential patients. Record the outcome of calls and make follow up calls as necessary. Enter referral information in system including clinical information and orders for services. Develop and maintain a working knowledge of all services and resources provided by Sentara and available within the community. Act as a resource for patient and referral source issues.

Education
- Highschool Graduate or Equivalent
- LPN
- Minimum 1 year of experience with clinical and customer service in a healthcare environment
- Home Health experience is highly preferred
