Sentara Medical Group is seeking to hire an individual to join our team as a Community Health Worker
Position Location: Harrisonburg, VA (rotation between clinic 8-5 pm and mobile bus)
Position Status: Full-time Days
Standard Working Hours: Monday - Thursday, 10:00 am -7:00 pm and Fridays, 8-5 pm (varies on mobile bus)
Night or weekend flexibility is needed for occasional community events.
Overview
Works in collaboration with the Case Manager and other healthcare team members to provide comprehensive and coordinated care. Engages in interventions to improve self-management skills and improve quality of life for patients with chronic and/or complex illness. Provides education and behavior modification strategies and provides social support to patients and families. Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management.

Education
- High School Diploma or Equivalent required
- Driver’s License-Certification-Other/National
- Bilingual Preferred English- Spanish
- Ability to write, type, and speak in preferred languages
- Experience working in a community-based setting preferred
- Experience working in community events and organizing events is preferred.
- Knowledge of medical terminology is preferred.
