What are the responsibilities and job description for the Care Manager position at Seniors Home Care?

As a Care Manager, you will play a key role in ensuring the safety, health and well-being of clients in the home environment. You will report directly to the Lead Care Manager.

Pay: $30-34/hr
Key Responsibilities:
- Collaborate with the Care Management team and staffing personnel to create effective care and staffing strategies.
- Conduct health and home assessments for new and prospective clients.
- Build relationships with prospects over the phone and schedule assessment visits.
- Provide orientation and ongoing training for caregiver team members, including specialized instruction when needed.
- Assess client needs and create individualized care plans that uphold our core values.
- Exercise strong clinical judgment and maintain a high standard of documentation.
Qualifications:
- Current, unencumbered Missouri RN, LPN license, MSW, or BSW with related experience
- CCM or CMC preferred but not required
- 2 plus years of experience working with geriatric patients/clients
- Valid driver's license with a good driving record
- Ability to work independently while providing excellent professional judgment
- Strong assessment, documentation, and careplanning skills
- Proficiency with Microsoft Office
