Our Ideal Candidate
We are seeking a dedicated and experienced Nurse to join our caregiving team in providing high-quality care for clients in Essex County, NJ . The ideal candidate will possess personal and/or professional background in home health, assisted living, and acute care settings, with a focus on developing and implementing effective care plans. As Home Health Agency Nurse, you will approve client care plans, oversee caregiver operations, ensure compliance with HIPAA regulations, and maintain high standards of client care through follow up home visits.
Job Responsibilities
Develop and implement comprehensive care plans tailored to individual client needs.
Conduct regular documentation reviews to ensure compliance with New Jersey regulatory standards and best practices.
Collaborate with interdisciplinary teams to enhance patient outcomes and quality of care.
Ensure adherence to MDS (Minimum Data Set) requirements for accurate patient assessments.
Maintain up-to-date knowledge of healthcare regulations and industry standards.
Foster a positive work environment.
Experience
A valid nursing license (RN) is required; BSN or higher preferred.
Proven experience in home health, assisted living, or acute care settings is essential.
Strong understanding of HIPAA regulations and their application in healthcare settings.
Familiarity with MDS processes and documentation review practices is a plus.
Excellent communication skills, both verbal and written, with the ability to collaborate effectively with diverse teams.
