Nurse Practitioner (NP) – Aging in Place Program
Seniors at Home LLC | Applicant May be Remote however preference given to candidates in Bend, Medford, Roseburg, and Newport, Oregon (Hybrid / Remote | Part-Time)
Seniors at Home helps older adults live safely and independently through personalized aging-in-place care management. We fill the gap between “too much care” and “not enough care” - supporting individuals who want to stay independent while ensuring the right level of clinical and wellness oversight.
We’re seeking a compassionate Nurse Practitioner (NP) to add to our team to provide clinical leadership for our Medicare-based programs, including chronic care management (CCM) and remote patient monitoring (RPM).
The NP will conduct virtual evaluations to determine Medicare eligibility, establish and update individualized care plans, and oversee patient progress through coordinated support. You’ll work alongside care managers, nurses, physical therapists, nutritionists, and family caregivers to help clients stay healthy and supported. “Home” may include private residences, assisted living, or independent living facilities. The NP also supervises care managers who perform patient outreach, vitals collection, and documentation, ensuring all care meets Medicare and clinical quality standards.
Qualifications:
· Oregon NP license (Family, Adult-Gerontology, or Primary Care)
· Compassionate, patient-focused, and detail-oriented
· Experience in geriatric care, primary care, or home health preferred
· Comfort working in a virtual, team-based care environment
· Proficiency in developing treatment plans and managing chronic conditions
· Experience in prescribing and adjusting medications and managing patient prescriptions
· Strong communication skills for patient and family education
· Ability to collaborate effectively with multidisciplinary healthcare teams
· Commitment to maintaining patient confidentiality and adhering to healthcare regulations
Compensation:
Hourly arrangement, based on experience and workload.
