At Seniors at Home, we are building a different model of healthcare, helping seniors safely remain independent at home through proactive care management, chronic care coordination, remote monitoring, and relationship-driven support. We fill the gap between "too much care" and "not enough care," supporting older adults who want to stay independent while ensuring the right level of clinical oversight.

We're expanding into Douglas County, and we're looking for a nurse to be Seniors at Home in your community: the person our Roseburg-area patients know, trust, and count on.

This is not traditional bedside nursing. This is relationship-based nursing with room to think, advocate, coordinate, and do the right thing for patients.

This is a focused role, not general nursing. You provide two services, Chronic Care Management (CCM) and Remote Patient Monitoring (RPM), and you coordinate with the patient's other providers, specialists, therapists, and caregivers who deliver the rest of their care. Much of the work is done remotely by phone and telehealth video from home, with periodic in-home visits to patients in your region.

What You Will Do

Chronic Care Management. Maintain an ongoing panel of senior patients: monthly care-management contact by phone and telehealth, periodic in-home visits, chronic condition monitoring, medication review, patient education, and coordination with each patient's providers and family. Identify changes in condition early to help prevent avoidable hospitalizations.

Remote Patient Monitoring. Patients use simple cellular health devices such as blood pressure cuffs, scales, and pulse oximeters that work without WiFi. You deliver and set up the devices during home visits, teach patients and families to use them, follow up on readings, and troubleshoot or replace units as needed. A central clinical team monitors incoming device alerts day to day; you provide the local follow-up with your patients.

Additional responsibilities:

• Field calls from referred and prospective patients, explain our services, and help them enroll.
• Conduct in-home eligibility screening and establishing visits, with a telehealth provider participating.
• Build and manage care plans and documentation in our care-management platform.
• Coordinate care across each patient's providers, specialists, therapists, caregivers, and family.
• Help develop workflows as the Roseburg hub grows.
• Participate in a shared on-call access structure.

The technology is consumer-level, not IT work. If you are comfortable using a smartphone or tablet for video calls and can teach patients to do the same, you have the technical skills needed. Our central team handles all system configuration and data.

Compensation

• $39 to $45 per hour, depending on experience and licensure (LPN or RN).
• Mileage reimbursed at the IRS standard rate for work-related travel.

Qualifications

• Active Oregon LPN or RN license (required).
• Based in or near Roseburg, with a reliable vehicle and willingness to travel for in-home visits within Douglas County (required). In-home visits, including initial establishing visits, may require up to a 60-minute drive from Roseburg.
• Experience with seniors, chronic conditions, care coordination, home health, hospice, Medicare patients, CCM, or remote monitoring is preferred.
• Comfortable using everyday consumer technology and teaching it to patients.
• Strong communication skills, dependable, and able to work independently.

Position Details

• Hybrid remote based in Roseburg, Oregon. Care-management calls and telehealth video are done from home; the role also requires periodic in-home patient visits within Douglas County.
• Part-time (about 15 hours per week) to start, with a defined path to full-time as the patient panel grows.
• Compensation and mileage as described above.

License: Oregon LPN/RN required. Location: Roseburg, OR area. Work location: hybrid remote in Roseburg, OR.
