Responsibilities
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility to promote comfort and independence.
- Monitor and record vital signs, including blood pressure, temperature, pulse, and respiration rates, accurately documenting all patient information in electronic health records (EHR systems).
- Support medication administration under supervision, ensuring proper techniques and adherence to safety protocols.
- Provide emotional support and companionship to patients and their families, respecting cultural and individual preferences.
- Maintain a safe environment by utilizing equipment like Hoyer lifts and adhering to aseptic techniques during patient care procedures.
- Collaborate with interdisciplinary teams including nurses, physicians, social workers, and therapists to coordinate comprehensive hospice services.
- Follow all HIPAA regulations and medical documentation standards to protect patient confidentiality and ensure compliance with healthcare laws.
Skills
- Proven experience in hospice care or related fields such as long-term care, assisted living, or nursing homes.
- Familiarity with EMR (Electronic Medical Records) systems like Epic or Cerner for accurate documentation.
- Knowledge of medical terminology, anatomy, physiology, and vital signs monitoring.
- Skilled in infection control practices including aseptic technique and infectious disease care protocols.
- Experience working with individuals with developmental disabilities or dementia care needs such as Alzheimer’s or memory care.
- Ability to perform complex patient care tasks including catheter care, phlebotomy support, and use of medical equipment like Hoyer lifts.
- Strong communication skills for effective patient service delivery and team collaboration in diverse healthcare settings including inpatient, outpatient, or home care environments.
