Position Summary
The Licensed Practical Nurse (LPN) provides compassionate nursing care to residents while ensuring their safety, dignity, and well-being. This role supports the Wellness Director in delivering high-quality care, supervising medication administration, and collaborating with staff, residents, and families in a homelike environment.
Key Responsibilities

Provide direct nursing care in accordance with resident care plans and physician orders.

Administer medications and treatments safely and accurately.

Monitor residents’ health status, report changes in condition, and document in accordance with state and facility guidelines.

Assist with admissions, transfers, and discharges, ensuring proper documentation.

Support, guide, and oversee PC Aides and Med-Techs to ensure quality care delivery.

Communicate effectively with residents, families, physicians, and interdisciplinary team members.

Participate in care plan meetings, audits, and quality assurance programs.

Respond promptly and appropriately to emergencies and urgent resident needs.

Uphold infection control, safety, and resident rights standards at all times.
Qualifications

Current and active Ohio LPN license in good standing.

Previous experience in assisted living, long-term care, or geriatrics preferred.

Strong clinical, critical-thinking, and communication skills.

Ability to supervise, train, and support direct care staff.

CPR certification and knowledge of basic first aid required.

Familiarity with electronic health records (PointClickCare preferred).

Must be dependable, flexible, and committed to resident-centered care.
