Overview
Senior Suites at St. Clair Commons is seeking a dedicated and compassionate Wellness Director to oversee the coordination and delivery of resident care services within our Personal Care community. This leadership role is responsible for ensuring exceptional standards of care, compliance with all regulatory guidelines, and fostering a positive, resident-centered environment through strong team leadership and clinical excellence.
Key Responsibilities
- Direct and oversee all aspects of resident care, ensuring services meet state regulations and company standards.
- Supervise, train, and support personal care aides and medication technicians to ensure high-quality service delivery.
- Conduct assessments, develop individualized care plans, and coordinate with physicians, families, and interdisciplinary teams.
- Admit, transfer, and discharge residents in accordance with established procedures; maintain accurate documentation in Point Click Care.
- Ensure compliance with Ohio Department of Health and Personal Care Home regulations.
- Participate in facility surveys, audits, and quality-assurance initiatives.
- Oversee medication administration systems and ensure compliance with state, federal, and company policies.
- Develop, implement, and evaluate departmental goals, policies, and procedures.
- Serve as a clinical liaison between residents, staff, families, and external healthcare providers.
- Maintain confidentiality of all resident information and ensure staff adherence to resident rights and privacy standards.
- Provide leadership through coaching, performance management, and staff education.
- Participate in ongoing professional development, required in-services, and safety programs.
Qualifications
- Current Ohio RN or LPN license in good standing.
- Minimum 3 years of experience in long-term care, geriatrics, or personal care home settings.
- Demonstrated leadership and supervisory experience in healthcare; department-head-level experience strongly preferred.
- Proficient in resident assessment, care planning, and electronic medical records (Point Click Care experience a plus).
- Knowledge of state regulations governing personal care and assisted living communities.
- CPR certified; strong communication, organizational, and critical-thinking skills.
- Valid driver’s license and ability to work flexible hours as needed.

- Maintain confidentiality of all resident information and protected health data.
- Treat all residents with dignity, kindness, and respect.
- Promptly report allegations of abuse, neglect, or misappropriation of property to the Administrator.
