Quality Assurance Nurse (Assisted Living – Ohio)

Overview
We are seeking a knowledgeable and detail-oriented Quality Assurance Nurse to oversee compliance and clinical quality across our two Assisted Living communities in Carrollton and St. Clairsville, Ohio.
This role is critical in ensuring that all resident care practices meet or exceed Ohio Department of Health (ODH) regulations and company standards. The ideal candidate will have a strong command of state laws, staff education, and quality-improvement processes, with the flexibility to travel between both communities as needed.
Key Responsibilities
- Conduct regular audits of care plans, medication administration, and documentation for compliance with Ohio Assisted Living regulations.
- Collaborate with community leadership to maintain survey readiness and proactively resolve compliance concerns.
- Develop and implement quality-improvement initiatives focused on resident safety, infection control, and care excellence.
- Provide training and education to nursing and personal-care staff on documentation, regulations, and clinical best practices.
- Review and analyze incident reports, track trends, and support corrective and preventive actions.
- Perform mock surveys and policy reviews to ensure readiness for ODH inspections.
- Partner with Administrators and Wellness Directors to promote consistent quality and performance between both sites.
- Maintain up-to-date knowledge of the Ohio Revised Code and Administrative Code governing Assisted Living operations.
- Serve as a clinical resource and mentor, fostering professional growth among care teams.
Qualifications
- Current Ohio RN or LPN license in good standing (required).
- Minimum 3 years of nursing experience in Assisted Living, Personal Care, or Long-Term Care settings.
- Previous experience in Quality Assurance, Compliance, or Survey Readiness strongly preferred.
- Comprehensive understanding of ODH regulations, infection control standards, and resident-rights requirements.
- Excellent communication, documentation, and analytical skills.
- Ability and willingness to travel regularly between Century Farms (Carrollton) and St. Clair Commons (St. Clairsville).
- Proficiency in electronic medical records systems (PointClickCare preferred).
- Demonstrated commitment to confidentiality, professionalism, and resident-centered care.
