At Senior Solutions (Area Agency on Aging in Southeastern Vermont), we are seeking a Case Manager to work with older adults and/or people with disabilities in Windsor, Vermont and the surrounding areas using a person-centered planning approach and a deep understanding of state and federal benefits. Strong organizational, communication, and computer skills are required.
Qualifications:
Bachelor’s degree in human services and prior case management experience preferred.
A combination of relevant education and experience may be substituted for a degree.

Must have reliable transportation and a valid driver's license.
Responsibilities include:
- Collaborating with other community partners to create a person-centered plan of care.
- Monitoring and reassessing program service plans as required.
- Having a working knowledge of services and programs available to older Vermonters and people with disabilities.
- Assisting, when necessary, to gain access to existing services such as public benefits, housing, legal services, etc.
- Ensuring that all provided services do not exceed budgeted amounts and are consistent with DAIL guidelines.
