Job Title: Care Coordinator

Job Duties:
- Program and Service Delivery: Provide intake and referrals. Complete intake assessments over the phone, in-person, and in-home. Input data into client tracking system. Assess seniors in their homes, makes referrals and help coordinate services. Develop service plan for each client and work with client towards achieving goals. Follow up with on-going visits and/or telephone calls. Maintain client flies. Maintain current knowledge of available services.
- Participate in Training: Attend supervision and support meetings as scheduled. Attend trainings in order to developed a good understanding of referral agencies, issues relating to aging and professional casework practices and as assigned.
- Community Relations: Promote and educate the community about the Care Watch Program. Attend town- ship, city and village meetings as assigned.
- Coordinate Services Within Senior Services: Work with RSVP to assist in linking volunteers with clients.
- Attend or supervise scheduled activities when needed and/or as assigned.
- Other duties as assigned or as deemed necessary under Supervisor and Executive Director.
Qualifications:
- General office skills with strong computer skills. Associates Degree preferred and/or experience with
older population preferred.
- Ability to work with a diverse population particularly those persons 60 years of age and older.
- Reliable Transportation for service area travel.
- Ability to sit and stand for long periods of time, able to push and pull tables and chairs, bending to
file cabinets, lifting objects of 50 lbs. or less.
- Good communications skills, able to work independently and well organized
