Role Description
This is a full-time, on-site position for a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) located in Austin, TX. we are looking for a geriatric care manager to join our team . the care manager will assess clients' needs, provide clinical care coordination, and psychosocial case management services to support older adults and their families. Responsibilities include connecting clients to community resources, conducting assessments and home visits, and collaborating with a multidisciplinary team to implement comprehensive care plans. The LMSW will also provide advocacy, crisis intervention, and accurate documentation while maintaining a compassionate and client-centered approach.

Qualifications
- Licensure as a Master Social Worker (LMSW) in the state of Texas
- Experience in geriatric care or working with older adults

- Ability to manage and document case files accurately and efficiently
- Problem-solving, crisis intervention, and emotional support skills
- Strong collaboration and teamwork capabilities within a multidisciplinary environment

- Proficiency in computer systems and familiarity with client management software
