General Description Of Role:
Provides professional case management to clients 60 years of age and older through:
- Interviewing clients and assessing needs.
- Completion of 6 Month Follow-Up home visits in accordance with Illinois Department on Aging and Senior Services Associates’ respective policies.
- Assisting clients in accessing needed services (internal and/or external).
Reports To:
Care Coordination Unit Supervisor
Education Requirements:
- Must be a RN, or a BSN or a BA/BS degrees in social science, social work, or related field. May substitute one year of program experience (defined as assessment, provision, and/or authorization of formal services for the elderly) for each year of college education.
- An LPN with one year of program experience (defined as assessment of and provision of formal services for the elderly and/or authorizing service provision).
- This requirement may be waived for persons hired/serving in this capacity prior to rule adoption (IDOA 220.605 a 1-4)
Job Duties & Responsibilities:
- Administer the appropriate intake forms, including a comprehensive needs assessment.
- Maintain complete and detailed case notes. This includes documentation of all client contacts, follow-up and of termination of services.
- Maintain all required department records and complete forms accurately and in a timely manner.
- Keep a daily log, maintaining records of appropriate billings and tracking time spent on each program.
- Develop and maintain a comprehensive knowledge of all relevant service and support programs needed to serve clients.
- Complete entirety of 6-month follow up process including scheduling of visits, completing the pre-visit checklist as well as the home visit, submitting billing documents and case recording, and completing reassessments as needed.
Other Job Requirements:
- Must have personal transportation and be willing to travel to client homes across multiple counties (Southern Kane and Kendall Counties).
- Applicant must be bilingual Spanish-speaking.
- Must be willing to attend training sessions and meetings where an occasional overnight stay may be required.
- Willing to enter into potentially uncomfortable physical settings to deal with clients.
