Community Resource Specialist
Senior Resource Center – Freeport, IL
The Senior Resource Center is seeking a compassionate, organized, and community-minded Community Resource Specialist to help older adults and their caregivers connect with programs, services, and resources that support independent living and enhance quality of life.
Position Summary
The Community Resource Specialist serves as a trusted resource for seniors and their families by assessing individual needs, providing information and referrals, assisting with benefit and service applications, and coordinating access to community resources. This position plays an important role in helping older adults remain safe, healthy, and independent.
Essential Responsibilities
· Meet with older adults and caregivers to assess needs and identify appropriate resources. Visits may take place in the office, community locations, or in client homes
· Provide information, referrals, and follow-up for community services and support programs.
· Assist clients with applications for benefits and assistance programs
· Maintain accurate and confidential client records and documentation.
· Build and maintain positive relationships with local service providers and community partners.
· Plan and participate in outreach events, educational programs, and community presentations.
· Advocate for older adults while promoting dignity, independence, and informed decision-making.
· Work collaboratively with Senior Resource Center staff to provide exceptional service.
· Complete InformUSA Certification and SHIP Counselor Certification within the first year or hire.
Qualifications
· Associate’s or Bachelor’s degree in Social Work, Human Services, Gerontology, Counseling, or a related field preferred; equivalent experience will be considered.
· Experience working with older adults, community services, or human service programs is preferred.
· Medicare and Medicaid knowledge preferred
· Excellent communication, interpersonal, and organizational skills.
· Ability to manage multiple priorities while maintaining attention to detail.
· Proficiency with Microsoft Office and basic computer applications.
· Valid driver’s license and reliable transportation.
