As we continue to grow, we are seeking an enthusiastic and relationship-oriented Community Referral Coordinator to help strengthen our partnerships throughout the healthcare community and support the continued growth of our organization.


Position Summary

The Community Referral Coordinator will work directly with the Chief Executive Officer to help develop and strengthen professional relationships with hospitals, physician offices, rehabilitation centers, skilled nursing facilities, Regional Centers, fiduciaries, discharge planners, and other community referral sources.


This is primarily an office-based position involving telephone outreach, scheduling appointments, maintaining referral relationships, coordinating marketing activities, and supporting community engagement initiatives. Occasional visits to referral partners and community organizations will be scheduled as relationship-building opportunities.


Primary Responsibilities

  • Develop and maintain relationships with hospitals, physicians, rehabilitation facilities, skilled nursing facilities, Regional Centers, fiduciaries, discharge planners, social workers, and community organizations.
  • Introduce healthcare professionals to the services offered by Senior Pro Services.
  • Conduct professional outreach visits and presentations within the community.
  • Identify new referral opportunities and develop long-term professional relationships.
  • Maintain regular communication with referral sources.
  • Represent Senior Pro Services at networking events, healthcare meetings, and community functions.
  • Provide regular activity and progress reports to the Chief Executive Officer.
  • Collaborate with leadership in implementing referral development strategies that support the continued growth of the organization.


Qualifications

  • Excellent interpersonal and communication skills.
  • Professional appearance and positive attitude.
  • Self-motivated, dependable, and well organized.
  • Comfortable working independently while maintaining regular communication with leadership.
  • Reliable transportation and willingness to travel throughout Northern California.
  • Experience in healthcare, community outreach, case management, customer relations, business development, or referral coordination is preferred but not required.
  • Knowledge of home healthcare or healthcare services is desirable.


Schedule & Work Location

  • Part-Time Position (15–20 hours per week)
  • Office-based at Senior Pro Services in San Leandro
  • Flexible work schedule
  • Occasional local community outreach and scheduled visits to healthcare providers and referral partners
  • Hourly Compensation: $35.00–$40.00 per hour, depending on qualifications and experience
