JOB DESCRIPTION:
In coordination with other Senior Network Services Programs, a Care & Housing Management Specialist provides care management services to clients in need of support. They make home visits to coordinate health and community support and other services to improve the quality of life and health. They provide information on available community-based options and services. They provide advocacy and make referrals to ensure that wrap-around services are used to help clients to remain healthy, housed and in place. They will make presentations to interested groups and others about services available to sustain Social Determinants of Health (SDOH).
GENERAL DUTIES & RESPONSIBILITIES:
Responds to requests for services, coordinates appropriate referrals to other community and health-focused resources, informs about services available to older adults and persons with disabilities. Suggests alternative methods of solving problems including mediation to remain housed.
Plans and schedules visits for older adults for the purpose of providing Care Management with special effort directed toward increasing independence at home and in the community. Help with encouraging activities to avoid social isolation.
Keeps required records and performs other clerical tasks as necessary.
Required to use a computer to prepare and develop a variety of documents, including records of contact, care management notes, invoicing, tracking, correspondence, reports, memos, brochures, flyers, program procedures; and copy and distribute as necessary.
Works in cooperation with other program teams.
Assists in monitoring program activity and promoting service availability.
Will report problematic situations to Management as necessary to secure maximum assistance for the client.
May attend community meetings regarding health and community supports issues.

MINIMUM QUALIFICATIONS:
B.A. degree with one year of experience in social services or A.A. with three years of applicable experience. The desired applicant will have experience specifically serving older adults and persons with disabilities.
Knowledge of older adult services, care management, including community services and public benefits.
Standard office practices and procedures, including filing, record-keeping, scheduling, and the use of standard office equipment and tech platforms including Google Suite, etc.,
Ability to deal tactfully and effectively with stakeholders, while being sensitive to their needs and concerns.
Organized, able to set priorities, use initiative and independent judgment, and maintain attention to detail is required.
The willingness and ability to work under supervision as well as independently is a must.
Physical stamina as required for standing, walking and climbing stairs while meeting with clients in their homes, carrying up to 35lbs.
HIGHLY DESIRED QUALIFICATIONS:
Experience with HIPAA.
Sensitivity to the needs of older adults and people with disabilities, and possess the ability to communicate clearly, creating an atmosphere of trust.
Bilingual English/Spanish is preferred.
OTHER REQUIREMENTS:
Valid driver’s license and fully insured vehicle necessary.
Fingerprinting and separate background check required.
Must provide a completed COVID-19 vaccination card, with or without accommodation.
