Registered Nurse (RN)

Position Summary
The Registered Nurse (RN) provides clinical oversight, assessment, and care coordination for
participants enrolled in waiver and in-home service programs. The RN ensures all services are
delivered safely, effectively, and in compliance with West Virginia Medicaid waiver guidelines
and organizational standards. This role supports participants in maintaining independence and
quality of life within their homes and communities.
Key Responsibilities
Clinical Assessment & Care Planning
● Conduct initial and ongoing nursing assessments for participants in FAIR, Lighthouse,
ADW, and Title 3E programs
● Develop, implement, and update individualized care plans based on participant needs
● Monitor participant health status and adjust care plans accordingly
Program Compliance & Documentation
● Ensure all services meet West Virginia Medicaid waiver regulations and program-
specific requirements
● Maintain accurate, timely, and compliant documentation
● Participate in audits and quality assurance activities
Care Coordination
● Collaborate with case managers, caregivers, physicians, and interdisciplinary team
members
● Provide clinical guidance to direct care staff
● Coordinate services to ensure continuity of care across programs
Supervision & Training
● Train and supervise personal care aides and other direct service staff
● Evaluate staff competency in delivering care
● Provide ongoing education on health, safety, and participant-specific needs
Participant Advocacy & Support
● Educate participants and families on health conditions, medications, and care plans
● Promote participant independence and dignity
● Identify and report concerns related to abuse, neglect, or exploitation
Health & Safety Monitoring
● Perform home visits to assess safety and care delivery
● Monitor infection control practices and medication management
● Respond to changes in participant condition and provide appropriate interventions
Qualifications
● Current, active Registered Nurse (RN) license in the State of West Virginia
● Minimum of 1–2 years of nursing experience (home health, geriatrics, or community
health preferred)
● Knowledge of Medicaid waiver programs (FAIR, Lighthouse, ADW, 3E) preferred
● Strong assessment, documentation, and care coordination skills
● Ability to work independently and as part of a multidisciplinary team
● Valid driver’s license and reliable transportation
