Key responsibilities include
- Providing direct, individualized care in a structured day program environment
- Assisting with personal care tasks such as bathing, grooming, dressing, and oral hygiene
- Supporting therapeutic exercises and activities as guided by on-site physical, occupational, and speech therapists
- Administering medications under the supervision of an on-site Registered Nurse (RN)
- Observing and reporting changes in patients’ conditions to RNs or therapists
- Promoting a safe and supportive environment, reporting any potential safety concerns
- Accurately documenting patient care and activity as required
- Performing additional duties based on individual care plans or team direction
Skills
- Strong ability to assist with personal care tasks while respecting client dignity and privacy.
- Familiarity with home care systems and medical terminology is preferred.
- Knowledge of CPR techniques and first aid procedures is beneficial.
- Experience in assisted living or home health environments is a plus.
- Excellent communication skills to effectively interact with clients, families, and healthcare teams.
- Ability to lift clients safely and perform physical tasks associated with caregiving.
- Strong organizational skills to manage time effectively while providing care.
