PRN Registered Nurse (RN) - Home Care Services

- Travel Required Between Myrtle Beach, SC & Conway, SC

Key Responsibilities
- Conduct client intake assessments, reassessments, and nursing evaluations
- Develop, implement, monitor, and update individualized care plans
- Ensure client needs remain aligned with care plans through ongoing evaluations
- Collaborate with management to maintain and improve quality care standards
- Utilize professional nursing judgment to assess physical, emotional, mental, and psychosocial client needs
- Serve as a clinical resource and mentor for caregivers and team members
- Advocate for client rights, dignity, and overall well-being
- Identify home safety concerns and reinforce safe care practices
- Address client concerns promptly and professionally while maintaining proper documentation
- Assist with onboarding, training, and support of caregivers and staff as needed
Qualifications
- Current and active RN license
- Bachelor of Science in Nursing (BSN) preferred
- Minimum of 1 year of long-term care experience preferred
- Ability to work independently while also contributing as part of a team
- Strong leadership, communication, and interpersonal skills
- Ability to supervise and support caregivers effectively
- Excellent organizational and multitasking abilities
- Commitment to maintaining confidentiality and professional standards
- Must maintain all CEU requirements in accordance with the Board of Nursing
