As our RN Case Manager, you will:
- Complete nursing assessments, care planning, and ongoing case management visits
- Evaluate client well-being and communicate key care insights to the internal care team
- Coordinate with leadership on quality initiatives and operational improvements
- Serve as a trusted clinical resource and mentor for caregivers
- Advocate for clients and uphold their rights with professionalism and compassion
- Manage and support our VA clients, ensuring exceptional service and outcomes
- Perform client assessments, including updating care plans and client dashboards in a timely manner
- Partner with the Scheduling team on caregiver assignments and schedule adjustments
- Review daily care logs to ensure care plans are followed
- Represent Senior Helpers at community/networking events
- Support training initiatives, including skills training, onboarding, and dementia training
- Contribute to the mission-driven goals of Senior Helpers through additional tasks as needed
- Active RN license (required)
- Valid driver’s license and auto insurance
- Strong communication, clinical judgment, and organizational skills
- Ability to work independently while being a collaborative team contributor
- Professionalism, patience, and adaptability in a fast-paced environment
- Commitment to treating clients and team members with dignity and respect
- A positive, solution-focused attitude
- Home care experience preferred, but not required—training provided
