Responsibilities:
- Conduct in-home assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor patient progress and adjust care plans accordingly
- Provide emotional support and education to patients and their families
- Valid LPN license in the state of Maine
- Prior experience in home health or a related field (preferred)
- Strong communication and interpersonal skills
