We are seeking a licensed clinician with a heart for patient advocacy and a passion for building professional relationships to join our growing team.
Key Responsibilities
- Build and grow relationships with hospitals, rehab centers, the VA and assisted living facilities.
- Collaborate with hospital and rehab discharge planners, case managers and physicians to identify patients suitable for home care.
- Provide education to patients and families about our home care services.
- Conduct clinical assessments to determine eligibility and service needs
- Represent Senior Helpers within the local senior community at events and programs.
- License as an RN, LPN, or LCSW (preferred)
- Experience in discharge planning or case management strongly preferred
- Strong interpersonal and communication skills
- Highly organized, self-motivated, and goal-oriented
- Valid driver’s license and reliable transportation
- Total Compensation: $65,000 - $110,000 (depending on license and experience)
- Bonus Structure: Performance-based bonuses tied to referrals and admissions
