The RN works closely with clients, families, and the interdisciplinary care team to ensure high-quality, safe, and compassionate care in a home-based setting.
Key Responsibilities
- Deliver direct patient care, including assessments, medication administration, treatments, and monitoring.
- Develop, implement, and update individualized care plans in collaboration with physicians and the care team.
- Monitor and document the client’s condition, reporting significant changes to the physician and agency.
- Provide patient and family teaching related to disease process, treatment, medications, and home care needs.
- Ensure infection control and safety practices are followed at all times.
- Coordinate with caregivers, therapists, and other healthcare providers to ensure continuity of care.
- Maintain accurate and timely clinical records in compliance with agency policies and state/federal regulations.
- Respond to client needs and emergencies with professionalism and clinical expertise.
- Current, active Registered Nurse (RN) license in the state of Maine.
- Minimum of [1-2] years of clinical nursing experience (home health or private duty preferred).
- Current CPR/BLS certification.
- Strong clinical assessment and critical thinking skills.
