Responsibilities
- Conduct scheduled client visits
- Assist with client assessments and ongoing monitoring
- Support implementation and updates of care plans
- Provide education and guidance to family caregivers
- Communicate client status changes to clinical leadership
- Support CNA caregivers through observation and feedback
Requirements
- Valid LPN license
- Experience in home care or a similar healthcare setting preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
