Registered Nurse-Home Health-Part Time

Job Overview
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being while promoting independence. This role requires a strong clinical background, excellent communication skills, and the ability to work independently in a home setting.

Duties

• Conduct start of care and recertification every 60 days or as needed.
• Conduct comprehensive patient assessments to develop individualized care plans.
• Administer medications and treatments as prescribed, including injections and IV therapy.
• Perform wound care, suture removal, and other necessary medical procedures.
• Assist with discharge planning and coordinate care with other healthcare professionals.
• Educate patients and their families about health management, medication administration, and lifestyle changes.
• Document patient progress and maintain accurate medical records using electronic health record systems such as Matrix
• Monitor vital signs.
• Utilize knowledge of anatomy to assess patient conditions effectively.

Requirements

• Current Registered Nurse (RN) license in the state of practice.
• Experience in home health or medical office settings preferred.
• Familiarity with electronic health record systems
• Understanding of discharge planning processes.
• Knowledge of anatomy is essential for effective patient assessment and care delivery.
• Excellent communication skills to interact with patients, families, and healthcare teams.
• Must be able to work independently while managing multiple patient needs.
