Role Description This is a full-time, on-site Primary Care Physician role based in Sacramento, CA, providing in-home medical care to patients. The physician will conduct comprehensive assessments, perform routine and urgent primary care visits, manage chronic conditions, and coordinate preventive care. Daily responsibilities include reviewing patient histories, ordering and interpreting diagnostic tests, prescribing medications, and developing individualized care plans. The role also involves collaborating with caregivers, specialists, and other members of the care team, maintaining accurate and timely documentation, and participating in quality improvement and care coordination initiatives. The physician will travel locally to patient homes, following a scheduled route supported by the Senior Doc administrative and clinical teams.
Qualifications
- Strong clinical background in Primary Care and Family Medicine, with the ability to manage a broad range of adult and geriatric conditions.
- Experience or training in Urgent Care and general Medicine, including evaluation and treatment of common acute issues in the home setting.
- Demonstrated competency as a Primary Care Physician, with a commitment to patient-centered, longitudinal care.
- Active and unrestricted medical license in the state of California; board certification or eligibility in Internal Medicine or Family Medicine.
- Current DEA registration and ability to prescribe medications in compliance with state and federal regulations.
- Previous experience with home-based care, geriatrics, or complex care management is highly beneficial.
- Proficiency with electronic health records (EHR), strong documentation skills, and comfort using mobile technology during visits.
- Excellent communication, cultural humility, and interpersonal skills, with the ability to collaborate effectively in an interdisciplinary team.
- Valid driver’s license, reliable transportation for local travel, and the ability to work independently in the field.
