About the job:

Pay: $77,400-$88,500

Monday-Friday 8:00 AM-4:30 PM, Occasional Weekends

The Social Work Supervisor Responsible for the direct supervision of social workers, behavioral health specialists, chaplain and social work case managers. Works under supervision of the Center Director while providing application of advanced social work knowledge and skills in participant screening, case management, counseling and referrals

Required Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.   • Master’s Degree from an accredited school of social work required
  • Be legally authorized, currently licensed, registered or certified if applicable in the state of employment.
  • LMSW Required
  • A minimum of two (2) years’ experience working with frail or elderly population required.
  • Must have a valid driver’s license, proof of insurance and have means of transportation
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact

Preferred Qualifications:

  • Experience working on a multi-disciplinary team in a hospital, nursing home or community-based setting is preferable

Essentials:

  • Conducts monthly department meetings to provide oversight and operational efficiency of the social work, behavioral health and spiritual care provided to program participants.
  • Continuously evaluate participants’ changing needs and make necessary programmatic adjustments to ensure social work, behavioral health and spiritual care interventions meet the needs of program participants.
  • Ensures job responsibilities, authorities and accountability of all direct reports are understood through one-on-one employee meetings or group supervisor.
  • Provides oversight and direct supervision of the social work department.
  • In coordination with the Quality and Compliance Team, provides oversight of Elder Abuse training for all new and current employees, ensuring compliance with regulations, policies and procedures related to elder abuse.
  • May carry a reduced panel of participants to support equity and efficiency of panel size across the organization.
  • Ensures in-person initial social work assessments are completed for Senior Community Care participants to obtain a complete psychosocial history, which may include descriptions of cognitive status, social supports, family dynamics mental health and substance dependency and other issues and needs. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
