What are the responsibilities and job description for the Adult Day Program Supervisor position at Senior Citizens of Kodiak, Inc.?
GENERAL FUNCTION: The person in this position manages and directs services offered through the Island Cove Adult Day Program. Person works closely with Deputy Director and CEO to administer the program. Person is responsible for ongoing review of the delivery of services to ensure quality of care provided attains the outcomes and goals of participants.
GENERAL RESPONSIBILITIES
1. Provide orientation, training and supervision to Island Cove Staff and Volunteers.
2. Implement all Island Cove Adult Day Policies and Procedures.
3. Ensure program meets requirements laid out by the State of Alaska grant for day-to-day operations (including staff to recipient ratios, meals, transportation, program services & activities)
4. Provide direct care to participants, leading by example for other staff.
5. Assist staff in completion of reports and records of service provided as needed.
6. Serve as activity coordinator
a. Plan, promote participation in, and supervise a variety of activities
b. Ensure activities are appropriate and meaningful in both individual and group settings
7. Ensure timely completion of admission, care plan and discharge for each participant.
8. Assist with maintenance of case records as directed, incorporating accepted practices of confidentiality and informed consent.
9. Ensure open and transparent communication is fostered between participants, family members and staff.
10. Provide outreach and coordinate efforts to seniors in need of and who have requested services.
11. Provide information and referrals to seniors of programs available, documenting and following up on referrals for services.
12. Coordinate with representatives of other agencies providing services for adult day program participants and care givers.
13. Assist with discharge plan for clients, utilizing available community resources to ensure a smooth transition if needed.
14. Maintain awareness of training needs, participate in training opportunities and comply with training requirements per State of Alaska grants.
15. Other related duties as requested by Deputy Director or CEO.
SKILLS, KNOWLEDGE AND ABILITIES:
Person in this position should have a Bachelor’s degree in social work or related field, or equivalent experience, good record keeping and oral/writing skills, experience with and
knowledge of local social service agencies. Must have at least one year of supervisory experience. CNA experience a plus.
Must be in good physical condition. Should be self motivated, dependable, organized and have previous experience with seniors, handicapped or disabled persons or related services. Must be able to relate to public, staff, community professionals and program participants and their families in a pleasant and tactful manner. Must be CPR and First Aid certified or able to become certified within the first 30 days.. Must have current Alaska driver’s license with good record.
WORKING CONDITIONS:
1. Exposed to body fluids, infection, odors, and behavior of participants.
2. Exposed to chemicals/cleaning solutions.
PHYSICAL DEMANDS:
1. Must be able to lift/push up to 50 pounds.
2. Must be able to lift, carry, stand, bend, squat, crawl, reach and kneels using good body mechanics for sustained periods of time.
3. Must possess sigh/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
4. Must be in good general health and demonstrate emotional stability in order to cope with the physical, mental and emotional stress of the position.
5. Must be able to move intermittently throughout the work day.
